We work as part of a large team of local Healthcare Professionals including, Consultants, GP's, District Nurses & Macmillan Nurses and others.

Any of these people may refer you to St Clare if they feel our services could help. If they do, it does not have to mean that you are about to die. They may just feel we could help you in some way now.

If you would like to find out more about us first, you can pop in and see one of the team or give us a call.

If you are happy to be referred, the health professional referring you, will fill out a form and send it with some details of your medical history to our Patient Services team. They will then choose the best service or services for you and contact you to discuss them.

Our team will then continue to work with your health professionals, keeping up to date with your treatment throughout the time you are a patient at St. Clare Hospice.

St Clare Hospice also regularly provide training to Healthcare Professionals to help them deliver the very best and latest treatment and care to their patients.
